Immunization Policy
Every student entering, attending or transferring to Fort Worth Country Day and every member of the School’s faculty and staff must be immunized against certain vaccine-preventable diseases as required by the Texas Department of State Health Services (TDSHS). Requirements for Students
Prior to entry, attendance or transfer to the School, every student must show acceptable evidence of the Required Vaccinations. The following documentation constitutes acceptable evidence: 1) documentation of vaccines administered that include the signature or stamp of a physician or his/her designee, or public health personnel; 2) an official immunization record generated from a state or local health authority; and 3) an official record received from school officials, including a record from another state.
Serologic confirmations of immunity to measles, rubella, mumps, Hepatitis A, Hepatitis B or varicella or serologic confirmation of immune response are acceptable in lieu of vaccine. Additionally, with regard to varicella (chickenpox) only, a written statement from a parent or legal guardian, school nurse or physician attesting to a child’s positive history of varicella disease or varicella immunity is acceptable in lieu of a vaccine record for that disease. Exclusions from compliance are allowable in Texas for public school students on an individual basis for certain reasons. However, the School shall deny enrollment to any student or prospective student who is considered excluded from compliance for any reason other than medical exemption. In order to claim a medical exemption, a student must have a letter from his or her physician stating that the Required Vaccine(s) would be medically contraindicated. This letter must be updated annually prior to the start of the School year. Students who receive such exemptions will be excluded from School during periods of emergency or epidemic as declared by the Head of School, in consultation with local health authorities.
Flu Vaccines for Faculty, Staff and Students
The School strongly recommends that all students and members of the faculty and staff receive an annual influenza vaccine.
